At Glenvale School, we often work closely with External Service Providers to ensure that students receive the support they need across all environments.
External Provider Approval
To proceed with providing therapy onsite at Glenvale School, please read the department of education guidelines (here) send through the following documentation as outlined below:
From your company we require:(if you're a sole trader, you will provide this)

Engagement Agreement
The Department has updated the engagement agreement, which is linked for your company to download, complete and sign.

Insurance Documentation
Please provide current copies (PDF) of the following:-
Professional Indemnity Insurance

Public Liability Insurance
-
Workers Compensation Insurance (or Personal Accident Insurance if you are a sole trader)

Mandatory Training Requirements
While the Department is in the process of updating training requirements, service providers are currently permitted onsite with:

Child Protection Awareness Induction Training, and
-
Emergency Care Training.

Screening Check
Please provide the therapist's WWCC number and date of birth so we can verify screening clearance with the Department.
Per student (from the therapist & parents) we require: (these are all found in the Engagement agreement and attached as separate documents to this email for your convenience)
5. Parent Request for Therapy Form
This form needs to be completed and signed by the student's parent/guardian. We’ll also be sending a hard copy home with the student, but a digital version is attached here for your reference. 6. Service Schedule

Please ensure that your SERVICE SCHEDULE matches the Parent Request for Therapy and the PLP Goals agreed on by teacher and parent: include Day/Time/duration under "service delivery schedule" and "Duration of service delivery" or write to be negotiated with by teacher. Include the wording "within line of sight of teacher" in location.
7. Attachment B - Form #1 (Parent Consent): This form must be signed by The student’s parent/guardian and must be submitted on your company letterhead.

8. After every session at school the therapist is to complete Form #2 - Therapy Update Form. The teacher will initial the completed form and then it will be handed in at the office.
Once all required documentation is received and reviewed, it will be forwarded to our principal for approval. If approval is granted, session days and times can then be arranged directly with the teacher at a time that suits the class.
To streamline the approval process, we recommend Allied Health Providers join the NSW Department of Education's Specialist Allied Health and Behaviour Support Provider Scheme.
